Prepare for Your First Visit
We'd like to take this opportunity to welcome you to Arizona Breast Consultants and to thank you for choosing our practice. We are constantly striving to make our patients' visits more efficient and more comfortable.
To prepare for your first visit, follow the information below:
1. Create an Account on our Patient Portal
This allows us to learn some basic information about you, and to communicate securely and privately with you. Once logged in, you can register for your first visit. Create Your Free Account
2. Print and Fill out the Medical History Form
This form helps us understand the nature of your current condition.
3. Prepare for your Office Visit
In addition to completing the above items, we will also require the following information for your fist visit:
- Referral or consultation form if applicable
- Prior X-Rays, MRI Films, CT Scans, or Mammograms
- Any pertinent medical history such as lab results or pathology reports
- Insurance card and any co-payment due for visit
- Please note: by law, minors must be accompanied by a parent or guardian
4. Get Directions to Our Office
Visit our Location page to find a map of the office where you will see your physician and get personalized driving directions.
